Cosmetic and Large Restorative Cases
To help serve you better, please help us by providing this checklist of the items we need to complete a cosmetic or large restorative case.
- Full arch model of pre-op
- Models of diagnostic wax up. You will receive a labial or tissue reduction prep guide with all diagnostic wax-ups.
- Photos of pre-op teeth with shade guide from different agles. (Place incisal of shade guide to incisal of tooth when taking shade)
- Photos of prepped teeth with stump shade tabs
- Full arch bite registration with (Futar-D)
- A bite taken with the upper provisional in place against the lower preparations
- A bite taken with the lower provisional in place against the upper preparations
- Stick bite: Verify that the incisal edge of the centrals is parallel with the interpupillary line.
- Photos of temporaries, full face including eyes from different angles, also eye to chin relaxed lip position.
- Models of temporaries, including the desired length of the final restoration
- Detailed lab RX expressing the aesthetic goals of the patient
- With all implant cases, indicate type and size of each implant to be restored
- Always call for an itemized case estimate
